On Sunday we had one new employee test positive at home. This employee had not been in the facility for 48 hours prior to testing positive. We have one remaining positive resident who will recover by the end of the week.

We have an additional 4 residents who tested positive on a rapid COVID test. We are awaiting the results of the PCR tests at this time. This brings our total count to 1 employee and 7 residents.
Our staff has been re-educated on proper use of PPE, we have begun deep cleaning of common areas and the Housekeeping Department has initiated frequent disinfecting protocols of high use items like door handles and hand rails.
I am also reminding all visitors that masks must be worn at all times in the hallways and they must be worn properly. We have masks at the front desk if you do not have one.

At this time, we have two COVID positive residents and one positive staff member. The two residents have been moved into isolation and the staff member is home completing their quarantine.
For now, we will be closing the Main Dining Room to limit the spread of the virus. Meals will be served in the rooms. Activities will continue, but each unit will alternate time in the day rooms to allow for cleaning in between.
We must ask that you do not visit if you are feeling under the weather or exhibiting symptoms of any kind. Please reconsider your visit if you have recently been around someone who has since tested positive.

I would just like to update everyone on our current COVID status. On June 30th our last COVID positive employee case was completely resolved. We have zero resident cases and zero employee cases as of now. We are waiting to hear back from the local Department of Health to officially end this outbreak.
Sadly, my last day at Imperial Healthcare will be Friday, July 22nd. I have thoroughly enjoyed my time here getting to work closely with the residents, staff and family members. During our routine outbreak testing, we had one additional staff member and one resident test positive for COVID. On Monday we conducted another round of COVID testing and all results came back negative. The patient who tested positive was sent out to the hospital for further evaluation. We do not have any positive cases in the facility at this time. Any fully vaccinated and boosted resident on Station One, low side is allowed to attend therapy in the gym. Those who are not boosted are required to quarantine for 7 days as per the state guidelines.
All residents are eligible for a second booster shot. We will be having our next COVID vaccine clinic on April 15th. You may call the nurses station or email to provide consent for a second booster for your loved one.

Yesterday, we had one resident test positive for COVID-19. Contact tracing found that this was a hospital exposure prior to discharge to Imperial. This resident has been in isolation since the day of admission. This one case does not put us in an outbreak status.
All other residents on Station one, low side have been tested and all are negative. All staff members that had any exposure are in the process of testing, but so far all are negative. For now, activities and therapy will be provided for in the rooms on station one, low side to limit any potential exposure to the rest of the facility.

Today’s testing confirmed that we are COVID free on Station 2 low and high sides and Station1 high side. We do still have one hospital acquired case on Station 1 low side. All staff members have fully recovered.
We will be allowing the residents from the three recovered units to join group activities and dining starting tomorrow. Visitation is allowed, but we ask those who have loved ones on Station 1, low side to still minimize visitation until we are completely recovered. For the most part our residents are experiencing mild symptoms. Our staff members are also reporting mild symptoms overall, and have tested negative anywhere between 5-8 days after a positive test. We will conduct another round of mandatory testing on Thursday.
I would like to thank the family members that have reported their positive test results to the Nursing Department. Your honesty helps us track and monitor your loved ones more closely. All departments have implemented increased cleaning schedules. All high touch surfaces and common areas are being cleaned multiple times per shift. Please understand that this variant is highly contagious and is unlike the strains we have seen so far. We are taking all precautions as brought forth by the CDC and Department of Health.
I want to assure you that throughout this outbreak we have been able to maintain our staffing levels and the care for your loved ones remains our top priority. We have paused on accepting any new admissions so that we can focus all care on our residents currently at Imperial.

During today’s mandatory round of testing we had 3 additional staff members test positive and three positive residents. Resident symptoms are very mild at this time. They will be treated with supportive care and we will address the symptoms as needed. This brings our total case count to 10 employees and 3 residents. We have one recovered staff member. Today we were notified that one resident and one employee have tested positive for COVID. This employee is not a direct caregiver and is asymptomatic, but will complete a quarantine at home. The resident has been moved to an isolation room.
All staff and residents will be tested today, December 27th and January 3rd. While this case does not stop or alter the visitation hours, we ask that you keep your mask on at all times during your visit. We are very happy to be able to hold regular visitation hours during this Thanksgiving holiday. However, we are respectfully requesting that, due to the high CALI score in our region, that no food be shared during your visit. Please fell free to bring food to leave for your loved one in a labeled and dated container.
Out on pass visits are allowed for the residents. We ask that you give the nurse’s stations advanced notice of an out on pass leave. To reduce the risk of exposure we are asking that residents remain masked during the car ride and around any unvaccinated individuals.
Please understand that we are asking this to remain COVID free at Imperial, especially as we approach the holiday season when visitation is at its highest.

Good afternoon,
Currently, we have one employee who tested positive last Saturday. This employee remains out of the facility on quarantine.
Today, we held another COVID vaccine clinic where we increased our number of vaccinated employees. We continue to be available to educate and counsel any employee or family member about the vaccine and booster.
It has been a great week of having our family members back to visit with no restrictions. We ask that you remember to screen in at the kiosk, wear proper face masks and refrain from traveling about the facility while visiting. If your loved one is not in his or her room, please go directly to the nurse’s station for assistance. We must still adhere to all precautions as we try to limit any exposure and spread of COVID-19.

Tuesday, November 16th, 2021 Dear Friends and Family, Today, we are very excited to announce that, according to new CMS guidelines, we are able to reopen to regular visitation from 8AM-8PM. These visits do not need to be scheduled. We will still require all visitors to be screened at our kiosk. If you have any symptoms such as fever, chills, sore throat, body aches or nausea, please refrain from visiting the facility until you have been free from these symptoms for at least 72 hours. Proper, medical grade face masks and face shields must be worn at all times. If you do not have one, we will provide you with one prior to your visit. All visitors must complete the COVID Information form prior to the visit. In order to keep our visitation open and unrestricted, we are asking that all visitors adhere to these safety measures. We are very happy to be back to our pre-COVID visitation hours and if we all work together, we should be able to remain this way for the foreseeable future. Have a great day! Today, we had an employee test positive for COVID. This employee did not have any direct contact with any residents and does not provide direct care.
We have tested all staff and residents and we can report that all tests have come back negative. At this time, we are still able to continue all normal visitation, dining and activities.

Dear Friends and Family,
We will be hosting a question and answer session with our Infectious Disease specialists on Wednesday, November 17th for residents and staff members in regards to the COVID vaccine. We will have our third COVID booster clinic on Friday, November 19th.
Just a reminder, face masks must be worn at all times while in the facility. Cloth masks are not considered face masks and can be exchanged for a new mask at the front desk.
For those listed as a Compassionate Care or Essential care visitor, please email imperialnjcommunications@gmail.com before 4 PM with your requested visit time for the next day’s visit. You do not have to wait for a response to your email. Please consider your request approved unless you get a call from the Activity Department with any concerns or conflicts.

We have been informed by our pharmacy that the Moderna and J&J booster shots will be available for our next clinic which will take place on Friday, November 19th. Please contact the Nursing Department if you have any questions regarding your loved ones’ booster status.
Our CALI score remains high for our area so we ask all visitors to please monitor any cold or flu-like symptoms prior to scheduled visitations. Please note that medical grade face masks must be worn at all times during indoor visits. Cloth face coverings cannot be worn in the facility.

On October 25th and 26th, we will be offering the flu vaccine to our employees. The flu shot is available to any employee or resident who requests it. All residents who have requested the flu shot have been vaccinated.

Dear Friends and Family,
We remain COVID free at Imperial Healthcare. Tomorrow, we will be hosting our first booster clinic for our staff members and those 65 and over who had previously received the Pfizer vaccine. We thank all of our family members who have submitted the necessary paperwork.
We will resume the flu shot vaccines one week after the booster shot is administered. For those of you who have yet to give consent for the flu shot, please fill out and submit the form below.
With a turn toward colder weather, our team has been actively working on some changes to our visitation program. We will have more details on these changes in next week’s update. We truly thank each one of you for working with us in trying to keep our residents safe.

Dear Friends and Family,
We are happy to report no new cases of COVID-19. Our second round of employee and resident testing was negative which will allow the residents in rooms 29-42 to come out of quarantine on August 28th and resume visitation and regular activities.
On August 25th, we had a large amount of dead trees removed from the front area of the facility. Now that the area is clear and safe, we will be dedicating those parking spots closest to the facility for visitor parking only. We ask that you park in these dedicated spots leaving the rest of the parking lot available for staff parking.
Again, I thank you for your cooperation and patience. We understand that these disruptions in visitation can be very frustrating, but please understand that these measures are put in place to protect the residents, our staff and our family members.

Attention All Staff:
On Monday, August 23rd, we will be conducting a second mandatory round of employee testing in order to open the rest of the facility to visits and activities. All employees must come to the facility to be tested even if you are not scheduled to work.
Starting Monday, August 23rd, all unvaccinated employees will be required to be tested twice a week. Testing will be conducted on Mondays and Thursdays. Please come to my office to be tested.
The regulation states that if we get a new positive case on the weekend, we have 48 hours to test all residents.and employees. A new case on a weekday gives us 24 hours to complete all testing. We realize that this can be inconvenient, but it will allow us to determine if there is any spread and It also allows us to reopen as quickly as possible if all cases are negative.
The CALI score is back to high for Monmouth County which means that eye protection must be worn on the units at all times . Please DO NOT report to work if you are experiencing any signs or symptoms of COVID.

Dear Family and Friends,
After a second round of testing of all shifts yesterday and today, we are happy to report no additional staff or resident cases. Our total at this time is 1 positive employee. This means that we can resume visitation and activities for residents in rooms 1-28 and rooms 43-56, tomorrow, August 19th. Unfortunately, residents in rooms 29-42 will have to remain on observation until August 29th.
Thank you for your patience during this time. In the case of a positive result, the New Jersey Department of Health requires us to pause visitation and immediately test all residents and employees to determine if there are any additional cases. We have to complete two full rounds of testing in order to determine that there was no further spread.
With our CALI score rising, we need to ask that ALL compassionate care visitors wear eye protection. We are also strongly encouraging ALL visitors to wear masks regardless of their vaccination status.

Today, we were informed that one of our staff members tested positive for COVID-19. This staff member is asymptomatic. We have already completed one round of testing of all residents and staff in the facility and all have tested negative.
Unfortunately, we will have to temporarily suspend all visitation, group activities and dining until we can complete a second round of testing on Tuesday, August 17th.

Dear Family and Friends,
Today we can report zero COVID cases in our facility. The previous case of a positive, per-diem staff member did not put our facility in an outbreak status because of the time between exposure and testing positive.
On Monday, we hosted another vaccine clinic where we added additional staff members to our vaccinated list. We continue to educate and encourage all staff members to get vaccinated, but those who are unvaccinated will continue to be tested weekly.
Thank you for your patience during the replacing of the roof. We are in the process of adding solar panels over the next few months. This project will be done in phases and we will make sure to communicate any changes to you on a weekly basis.

We are happy to report that we have had no additional reports of COVID at Imperial. Our total case count remains at 1 employee. Yesterday, we completed another round of testing so that we can continue to keep everyone safe.

Dear Family and Friends,
We are happy to report that after two consecutive rounds of testing, all residents and staff have tested negative, keeping our COVID case count to 1 Per Diem employee.
This means that visitation, activities and dining can resume for the unaffected units in the facility (rooms 16-56). The residents on the exposed unit, Station One Low (rooms 1-14), will remain in a temporary quarantine until August 2nd. This will be the protocol as we move forward with the presence of the more contagious Delta variant and experience breakthrough cases.

Today we were informed that a member of our per-diem nursing staff tested positive on a rapid COVID antigen test. We have completed one full round of testing of our residents and all tests have come back negative. We are in the process of testing all employees, and all who have been tested thus far have been negative.
On Monday we will conduct our second round of testing. Unfortunately, we are required to put a temporary pause on our visitation, dining and activity programs.
Once the second round of testing has been completed, we will communicate any changes or updates to you via Constant Contact.

Happy Friday on this hot and humid day. We continue to remain COVID free here at Imperial. Today, we are hosting another vaccine clinic to further protect our residents and staff against the virus. In an attempt to take all precautions against the new Delta variant, we will be instituting a new policy starting Monday, July 19th. All staff members, regardless of vaccination status, who travel out of state, will be tested for 7 days upon returning to work. Please understand that this for the safety of everyone.
On Monday, July 19th, we will be kicking off our Imperial Healthcare Olympic Games. The residents and staff will be divided into two teams and will face off in a variety of competitions throughout the week. Dear Family and Friends,
We are happy to report another week of activity here at Imperial Healthcare. We continue to hold indoor and outdoor visitation. Please be mindful of the heat index. If the heat index rises above 80 degrees, we will be moving the visits, to designated areas inside of the facility. Please remain in your designated areas until the Activity Department accompanies you back to the lobby area.
We will be hosting a 4th of July barbecue for our residents and staff on Sunday. Bob and the Food Service Department have a delicious menu planned. Please follow us on FaceBook and Instagram for current updates and photos of our facility events.
On Monday, June 28th the Activity Department opened the Imperial Emporium where residents can use their Royal Rewards Card to spend points that they accumulate while attending activities. Wednesday, March 24th, 2021 Dear Family and Friends, Our weekly Covid update includes two staff members. One of the two employees will be cleared to return to work tomorrow. We have ZERO positive residents at this time. We are happy to report that our pharmacy will be be able to distribute the vaccine to our residents and staff in the near future. We have another large group of employees who will be vaccinated along with a small group of residents. These additional residents will bring our resident vaccination percentage to just under 100%. We understand that everyone’s biggest question is when can indoor visitation resume. The answer is as long as the remainder of Monday’s PCR tests and today’s rapid tests come back with a negative result, we will be able to resume indoor visits on Tuesday, March 30th. If we confirm no new positive cases, a link will be sent on Monday for you to schedule your INDOOR visit. However, we are happy to announce that tomorrow we will begin outdoor visitation, weather permitting. Please use the link at the bottom to schedule your OUTDOOR visit. If you already have a window visit scheduled for tomorrow, it will now become an outdoor visit as long as the resident consents. Please note that residents on a 14 day precautionary isolation are not eligible for a visit until their 15th day. Visits are still limited to two visitors per visit and one visit per week, per resident. EVERY visitor must arrive 15 minutes prior to your scheduled time to be properly screened, given proper PPE and be rapid tested. Rapid testing is not required, but we are asking all families to consent to testing as it will greatly reduce the risk of future COVID spreads within the facility. A resident who is considered fully vaccinated and gives full consent and is capable of proper hand hygiene will be allowed to be hugged and kissed by their family members and visitors with proper PPE. Social distancing measures will still be in place for those residents who have not been fully vaccinated. Wednesday, March 17th, 2021 Dear Family and Friends, During our weekly testing protocols, we learned of one additional staff member testing positive for COVID-19. We will continue the mandatory resident and employee testing. This gives us a total of 0 residents and two staff members positive for COVID-19 For the next week we will be adding a third day of employee testing. We have taken this additional measure upon ourselves in order to mitigate any additional spread of the virus. We understand that there are many questions as to the level of infection in our area and how that impacts visitation. Please take a look at the link below to better explain the CALI score and how those decisions are made by the state and local Departments of Health. We are considered part of the Central East Region which, as of the last report, still had us listed as high for activity level. Unfortunately, at this time, only end of life visits are allowed. We are aware of the newly published CMS guidelines, however, we will not be able to proceed until we receive further clarification from the New Jersey Department of Health. Saturday, November 28th, 2020 Dear Family and Friends, On Tuesday, November 24th, we conducted another round of mandatory testing of all residents and staff members. Additional results have informed us that we have , two additional employees testing positive for COVID-19 and one additional resident. The employees and the resident are asymptomatic. This gives Imperial a total of 5 employees and 6 residents who have tested positive for Covid-19 as one resident has recovered. All staff members are quarantining at home and all residents have been isolated. All residents and employees are asymptomatic at this time. On Wednesday, November 25th, representatives from the New Jersey Department of Health conducted a one day, Infection Control survey. They examined all aspects of infection control and spent time observing caregivers on the COVID-19 unit. They had zero findings and awarded us a second deficiency free survey. Although we are extremely proud of the hard work done by the Imperial Team, we feel that we can still take additional measures to assist in mitigating the spread of this virus. Therefore, we have increased the housekeeping and sanitizing of all common areas including nurses stations, break rooms, med carts, door knobs and restrooms. We will also be reviewing hand washing, PPE usage and all other infection control measures with all staff. We continue to follow all CDC, CMS and State Department of Health guidelines. We have been conducting additional, interim rapid testing in between our weekly testing requirements when necessary. We will complete another round of PCR testing of all staff Monday and Tuesday. Monday, November 23rd, 2020 Dear Family and Friends, On Thursday, November 19th, we conducted our second round of testing of all residents and staff members to establish a new baseline. From that round of tests, one additional employee tested positive for COVID-19. We immediately began testing any resident that the employee has come in contact with. The rapid testing of those residents revealed one additional resident who also tested positive. This gives Imperial a total of 2 employees and 6 residents who have tested positive for Covid-19. Both staff members are quarantining at home and all residents have been isolated. All residents and employees are asymptomatic at this time. We continue to follow all CDC, CMS and State Department of Health guidelines. We have been conducting additional, interim rapid testing to attempt to mitigate the spread in between our weekly testing requirements. We will complete another round of PCR testing tomorrow of all residents and staff. We will be hosting a second Family Conference Call on Wednesday, November 25th at 11am. Dear Family and Friends, During our routine, weekly testing, we were notified of a positive COVID-19 result in one of our long-term care residents. At this time, the resident is asymptomatic and has been placed on isolation. All residents and staff will be tested within 24 hours and then a second round of testing will be completed on Thursday to establish a new baseline. Unfortunately, we will have to suspend all indoor, outdoor and window visitation until further notice. FaceTime and Skype calls can be scheduled by using the imperialnjcommunications@gmail.com address. We will be in contact with our local Department of Health for all guidance and safety measures and will continue to follow all State Department of Health and CMS guidelines. Ensure sufficient supply of Hand Hygiene resources in the facility.
Evaluate personal protective equipment (PPE) and report level to corresponding agency/OEM/LDOH/NJDOH.
Educate staff on infection prevention practices, control measures for COVID-19.
Facility will implement Universal Source control by providing facemask to person in the facility. As an alternative, cloth mask will be provided in the event of supply shortage for staff that does not provide direct care to the residents.
Daily Reporting
Complete line listing for residents / staff.
Submit line listing to Monmouth County DOH per instruction. Monmouth County representative Enrico Cabredo.
Admission, Transfer and Readmission
Consider closing to new admissions if the facility is unable to appropriately cohort. This does not include readmission back to the facility.
Close the unit affected to new admissions except as needed to cohort individuals and staff.
The facility will coordinate with the transport company if a presumptive or confirmed COVID-19 is going to be transported. Face mask will be provided to the resident for transport to protect the transport staff and receiving facility staff.
The facility physician will coordinate with the receiving facility’s physician to ensure report is accurately communicated and precaution is in place.
Dedicate equipment in isolation rooms. If not possible, clean and disinfect equipment using the EPA-registered disinfectant before use with another resident within the cohort.
Conduct routine cleaning and disinfection of high touch surfaces and shared medical equipment using EPA-registered disinfectant.
Increase frequency of routine cleaning and disinfection.
Prioritize rounding in a “well to ill’ flow to minimize cross-contamination.
New admissions and readmission will be monitored for signs of COVID 19 for 14 days and will be cared for by staff using COVID-19 PPE according to CDC guidelines. They will be placed in private rooms (if available). If feasible, a cohort will be created for this population.

Restrict visitors and non-essential healthcare personnel, except in certain compassionate-care situation.
Evaluate all persons who enter the facility for signs of COVID-19. Temperature checks at entry point. Temperature checks at the start of each shift for all employees.
The facility will monitor any visitors/vendors/physician for temperature and any signs of respiratory symptoms. The following will also be assessed: history of international travel with 14 days, history of exposure to a presumptive or positive COVID-19.
Visitors with respiratory symptoms will not be permitted to enter the facility at any time.
Facility will require visitor who are cleared to enter the facility will be asked to perform hand hygiene and use of facemask and restrict visitation to resident’s room.
Signage for visitation, cough etiquette and hand hygiene will be posted in several location within the facility.
Visitation restriction will be communicated through email, website and posting of signage
Active screening for residents for fever and other COVID-19 symptoms every shift.
Any supply delivery will be dropped off by the loading dock and/or right by the reception area.
Alcohol-based hand sanitizers for hand hygiene will be provided in the hallways, in each resident’s room, medication carts and throughout the building.
PPE needed will be made available outside the doors for easy access to the staff.
Facility will provide EPA disinfectants to allow for frequent cleaning of high touched surfaces and shared resident equipment.
Suspend communal dining and all group activities.
Residents are encouraged to remain in their rooms. If the resident leaves their room, they should wear a face covering or face mask; perform hand hygiene, limit movement in the facility and perform social distancing.

In the event staffing cannot be designated specifically in the COVID-19 unit, Staff will work from clean unit to COVID Unit., Staff will cluster work load from clean resident to COVID residents.
Staff will don COVID-19 recommended PPE according the CDC guidelines.
Laboratory confirmed COVID-19 positive residents will be relocated to designated cohort. Private rooms will be provided if feasible. Cohort with another COVID-19 resident maybe done when private room is not possible.
Person Under investigation will not be moved to COVID-19 cohort until results are confirmed.
Roommates of COVID-19 residents is considered exposed and should not share a rooms with others unless they have tested negative and has been asymptomatic for 14 days from last exposure.
Facility will adhere to the CDC guideline for the discontinuation of Transmission-Based Precaution.
Symptom-based strategy
At least 3 days (72 hours) have passed since recovery defined as resolution of fever without the use of fever-reducing medications and improvement in respiratory symptoms (e.g., cough, shortness of breath); and,
At least 10 days have passed since symptoms first appeared

Enforce use of universal facemask (source control)for staff while in the facility in addition to active screening of symptomatic staff.
