We are happy to announce that all of our residents are fully recovered as of today. We still have one staff member who is quarantining at home. Thank you for your patience and cooperation over the last few weeks. We will be opening the dining and activities for both units.

Dear Family and Friends,

On Sunday we had one new employee test positive at home. This employee had not been in the facility for 48 hours prior to testing positive. We have one remaining positive resident who will recover by the end of the week.

All other residents and employees have completely recovered. Thank you for your patience during this last outbreak. Dear Family and Friends,

We have an additional 4 residents who tested positive on a rapid COVID test. We are awaiting the results of the PCR tests at this time. This brings our total count to 1 employee and 7 residents. 

Our staff has been re-educated on proper use of PPE, we have begun deep cleaning of common areas and the Housekeeping Department has initiated frequent disinfecting protocols of high use items like door handles and hand rails.

I am also reminding all visitors that masks must be worn at all times in the hallways and they must be worn properly. We have masks at the front desk if you do not have one.

We have one additional resident who tested positive for COVID-19 bringing our total to three residents and one staff member. All are exhibiting mild symptoms.

Dear Family and Friends,

At this time, we have two COVID positive residents and one positive staff member. The two residents have been moved into isolation and the staff member is home completing their quarantine.

For now, we will be closing the Main Dining Room to limit the spread of the virus. Meals will be served in the rooms. Activities will continue, but each unit will alternate time in the day rooms to allow for cleaning in between. 

We must ask that you do not visit if you are feeling under the weather or exhibiting symptoms of any kind. Please reconsider your visit if you have recently been around someone who has since tested positive.

We have had two employees test positive for COVID-19 who are currently at home in quarantine. We also have one additional positive resident for a total of 2 residents. Both of these residents are in isolation.

We are strongly asking that any and all visitors adhere to the following guidelines:

  1. When visiting a resident, remain in the room with the resident. Please do not visit in common areas a this time.
  2. Please leave your mask on unless you and the resident are the only people in the room.
  3. DO NOT plan a visit if you have any symptoms of any kind or if you have been around someone who has been exhibiting symptoms.

We had one resident test positive for COVID. We have one other resident on precautionary isolation. We have zero staff members positive at this time. The COVID cases are starting to rise again in Monmouth County and around the state. We strongly encourage any and all visitors who are exhibiting any signs or symptoms of illness to please refrain from visitation at this time.

Dear Friends and Family,

I would just like to update everyone on our current COVID status. On June 30th our last COVID positive employee case was completely resolved. We have zero resident cases and zero employee cases as of now. We are waiting to hear back from the local Department of Health to officially end this outbreak.

Sadly, my last day at Imperial Healthcare will be Friday, July 22nd. I have thoroughly enjoyed my time here getting to work closely with the residents, staff and family members. Mr. Marc Schon, the Executive Director, will be taking over the role of Administrator as of July 23rd. I wish all of you health and happiness in the future.

We are happy to report that we have no new cases of COVID-19 at this time. We currently have one positive patient with no symptoms. All employees and other residents have fully recovered.

Dear Family and Friends,

During our routine outbreak testing, we had one additional staff member and one resident test positive for COVID. That brings us to 2 employees and one resident positive with 2 employees and 2 residents recovered. We will continue outbreak testing until we have two weeks of consecutive negative tests for all employees and residents.

Yesterday, we had one additional staff member test positive for COVID-19. This employee was asymptomatic and was sent home to quarantine. Today, our two positive residents and one isolation resident will return to their rooms, ending their COVID status.

Yesterday, we were informed that a staff member had a positive RAPID COVID test result. The employee is now at home in quarantine. The facility will follow the NJDOH guidelines on testing and all other infection control measures.

Dear Friends and Family,

On Monday we conducted another round of COVID testing and all results came back negative. The patient who tested positive was sent out to the hospital for further evaluation. We do not have any positive cases in the facility at this time. Any fully vaccinated and boosted resident on Station One, low side is allowed to attend therapy in the gym. Those who are not boosted are required to quarantine for 7 days as per the state guidelines.

All residents are eligible for a second booster shot. We will be having our next COVID vaccine clinic on April 15th. You may call the nurses station or email to provide consent for a second booster for your loved one.

March 31st, 2022

Dear Family and Friends,

Yesterday, we had one resident test positive for COVID-19. Contact tracing found that this was a hospital exposure prior to discharge to Imperial. This resident has been in isolation since the day of admission. This one case does not put us in an outbreak status.

All other residents on Station one, low side have been tested and all are negative. All staff members that had any exposure are in the process of testing, but so far all are negative. For now, activities and therapy will be provided for in the rooms on station one, low side to limit any potential exposure to the rest of the facility.

Today’s testing confirmed that we are COVID free on Station 2 low and high sides and Station1 high side. We do still have one hospital acquired case on Station 1 low side. All staff members have fully recovered.

We will be allowing the residents from the three recovered units to join group activities and dining starting tomorrow. Visitation is allowed, but we ask those who have loved ones on Station 1, low side to still minimize visitation until we are completely recovered. We are still asking all family and friends to refrain from visiting if you are exhibiting symptoms of any kind.

For the most part our residents are experiencing mild symptoms. Our staff members are also reporting mild symptoms overall, and have tested negative anywhere between 5-8 days after a positive test. We will conduct another round of mandatory testing on Thursday.

I would like to thank the family members that have reported their positive test results to the Nursing Department. Your honesty helps us track and monitor your loved ones more closely. All departments have implemented increased cleaning schedules. All high touch surfaces and common areas are being cleaned multiple times per shift. Please understand that this variant is highly contagious and is unlike the strains we have seen so far. We are taking all precautions as brought forth by the CDC and Department of Health.

I want to assure you that throughout this outbreak we have been able to maintain our staffing levels and the care for your loved ones remains our top priority. We have paused on accepting any new admissions so that we can focus all care on our residents currently at Imperial.

Over the weekend we had 7 residents test positive for COVID-19 and four additional staff members test positive. This brings our current total to 18 staff members, 2 recovered staff members and 11 positive residents with 1 recovered. We will have another round of mandatory testing for all residents and staff members on Monday. All symptoms remain mild and the recovery period in staff members looks to be about 5-8 days so far.

Last night we had one resident test positive for COVID-19 and today we had one additional staff member test positive. We do have another recovered employee so that brings our current total to 10 staff members, 2 recovered staff members and 4 residents. All residents were tested again today and we do not have any additional cases beyond the four stated. Resident symptoms remain mild. We will have another round of mandatory testing for all residents and staff members on Monday.

Dear Family and Friends,

During today’s mandatory round of testing we had 3 additional staff members test positive and three positive residents. Resident symptoms are very mild at this time. They will be treated with supportive care and we will address the symptoms as needed. This brings our total case count to 10 employees and 3 residents. We have one recovered staff member. If your resident tests positive, you will receive a call from the Nursing Department. Please contact the Nursing Department if you or a family member tests positive for COVID and have visited your loved one in the last 14 days.

We will be closing the dining room and group activities for a brief time while we try to get past this current outbreak. For now, residents will have one on one visits from the Activity Department and meals in their rooms. Visitation is still allowed, but we strongly encourage the use of virtual visits at this time, especially if you are not feeling well. During our second round of mandatory testing, we had five additional staff members test positive for COVID-19. These are non-direct caregivers and bring our total of positive staff cases to 6. We have zero resident cases at this time. We will have another round of mandatory testing on January 3rd.

At this time we are going to move all family and Interdisciplinary meetings to phone conferences. We continue to ask that you to wear a proper face mask during your visits and use frequent and proper hand washing techniques at all times.

The PCR tests on both the resident and staff member that tested positive on Tuesday have come back negative, however, we had one other employee test positive on a rapid test. This employee is not a direct caregiver. We will update you should the situation change.

We understand that everyone would like to see their loved ones over the holidays and we are glad that there are no restrictions, but we ask that you please take all precautions necessary to protect yourselves and the residents and staff. Please use frequent hand washing, appropriate face masks and wipe all high touch surfaces indoor home. Please stay home if you have any symptoms. These include cold, flu and sinus symptoms. The COVID case count in New Jersey is at its highest ever and the entire state is in the High category on the CALI score. We want to keep everyone safe and healthy, especially over the holidays. Dear Friends and Family,

Today we were notified that one resident and one employee have tested positive for COVID. This employee is not a direct caregiver and is asymptomatic, but will complete a quarantine at home. The resident has been moved to an isolation room.

All staff and residents will be tested today, December 27th and January 3rd. While this case does not stop or alter the visitation hours, we ask that you keep your mask on at all times during your visit. We also ask that you do not come to the facility if you are having symptoms of any kind. This includes mild symptoms such as runny nose, sore throat or mild cough.

It has been a pleasure to see so many of you return to Imperial Healthcare for unrestricted visitation. While we are very happy to have you here, we need to make everyone aware of the fact that the CALI score for our region remains high. With the high CALI score and the unknown effects of the Omicron variant, we are asking the following:

  1. All family members, friends and visitors REFRAIN from visiting if experiencing any symptoms of any kind including cold, flu and especially COVID symptoms.
  2. Please postpone your visit if you learn that you have been around a symptomatic person until you can confirm that you have not been infected.
  3. PLEASE make the facility aware if you or another visitor in your family tests positive so that we can take immediate steps to attempt to limit the exposure and possible spread.
  4. Please wear a proper face mask and use good hand hygiene before and after visits.

Please understand that we are being overly cautious to protect your loved ones , our staff and our visitors. Good Afternoon,

We are very happy to be able to hold regular visitation hours during this Thanksgiving holiday. However, we are respectfully requesting that, due to the high CALI score in our region, that no food be shared during your visit. Please fell free to bring food to leave for your loved one in a labeled and dated container.

Out on pass visits are allowed for the residents. We ask that you give the nurse’s stations advanced notice of an out on pass leave. To reduce the risk of exposure we are asking that residents remain masked during the car ride and around any unvaccinated individuals.

Currently, we have one employee who tested positive last Saturday. This employee remains out of the facility on quarantine.

Today, we held another COVID vaccine clinic where we increased our number of vaccinated employees. We continue to be available to educate and counsel any employee or family member about the vaccine and booster.

It has been a great week of having our family members back to visit with no restrictions. We ask that you remember to screen in at the kiosk, wear proper face masks and refrain from traveling about the facility while visiting. If your loved one is not in his or her room, please go directly to the nurse’s station for assistance. We must still adhere to all precautions as we try to limit any exposure and spread of COVID-19.

Dear Friends and Family,

Today, we are very excited to announce that, according to new CMS guidelines, we are able to reopen to regular visitation from 8AM-8PM. These visits do not need to be scheduled.

We will still require all visitors to be screened at our kiosk. If you have any symptoms such as fever, chills, sore throat, body aches or nausea, please refrain from visiting the facility until you have been free from these symptoms for at least 72 hours. Proper, medical grade face masks and face shields must be worn at all times. If you do not have one, we will provide you with one prior to your visit. All visitors must complete the COVID Information form prior to the visit.

In order to keep our visitation open and unrestricted, we are asking that all visitors adhere to these safety measures. We are very happy to be back to our pre-COVID visitation hours and if we all work together, we should be able to remain this way for the foreseeable future.

We will be hosting a question and answer session with our Infectious Disease specialists on Wednesday, November 17th for residents and staff members in regards to the COVID vaccine. We will have our third COVID booster clinic on Friday, November 19th.

Just a reminder, face masks must be worn at all times while in the facility. Cloth masks are not considered face masks and can be exchanged for a new mask at the front desk.

For those listed as a Compassionate Care or Essential care visitor, please email imperialnjcommunications@gmail.com before 4 PM with your requested visit time for the next day’s visit. You do not have to wait for a response to your email. Please consider your request approved unless you get a call from the Activity Department with any concerns or conflicts.

We remain COVID free at Imperial Healthcare. Tomorrow, we will be hosting our first booster clinic for our staff members and those 65 and over who had previously received the Pfizer vaccine. We thank all of our family members who have submitted the necessary paperwork.

Dear Friends and Family,

We are happy to report no new cases of COVID-19. Our second round of employee and resident testing was negative which will allow the residents in rooms 29-42 to come out of quarantine on August 28th and resume visitation and regular activities.

On August 25th, we had a large amount of dead trees removed from the front area of the facility. Now that the area is clear and safe, we will be dedicating those parking spots closest to the facility for visitor parking only. We ask that you park in these dedicated spots leaving the rest of the parking lot available for staff parking.

Again, I thank you for your cooperation and patience. We understand that these disruptions in visitation can be very frustrating, but please understand that these measures are put in place to protect the residents, our staff and our family members.

Attention All Staff:

On Monday, August 23rd, we will be conducting a second mandatory round of employee testing in order to open the rest of the facility to visits and activities. All employees must come to the facility to be tested even if you are not scheduled to work.

Starting Monday, August 23rd, all unvaccinated employees will be required to be tested twice a week. Testing will be conducted on Mondays and Thursdays. Please come to my office to be tested.

The regulation states that if we get a new positive case on the weekend, we have 48 hours to test all residents.and employees. A new case on a weekday gives us 24 hours to complete all testing. We realize that this can be inconvenient, but it will allow us to determine if there is any spread and It also allows us to reopen as quickly as possible if all cases are negative.

The CALI score is back to high for Monmouth County which means that eye protection must be worn on the units at all times . Please DO NOT report to work if you are experiencing any signs or symptoms of COVID.

Dear Family and Friends,

After a second round of testing of all shifts yesterday and today, we are happy to report no additional staff or resident cases. Our total at this time is 1 positive employee. This means that we can resume visitation and activities for residents in rooms 1-28 and rooms 43-56, tomorrow, August 19th. Unfortunately, residents in rooms 29-42 will have to remain on observation until August 29th.

Thank you for your patience during this time. In the case of a positive result, the New Jersey Department of Health requires us to pause visitation and immediately test all residents and employees to determine if there are any additional cases. We have to complete two full rounds of testing in order to determine that there was no further spread.

With our CALI score rising, we need to ask that ALL compassionate care visitors wear eye protection. We are also strongly encouraging ALL visitors to wear masks regardless of their vaccination status.

Today, we were informed that one of our staff members tested positive for COVID-19. This staff member is asymptomatic. We have already completed one round of testing of all residents and staff in the facility and all have tested negative.

Dear Family and Friends,

Today we can report zero COVID cases in our facility. The previous case of a positive, per-diem staff member did not put our facility in an outbreak status because of the time between exposure and testing positive.

On Monday, we hosted another vaccine clinic where we added additional staff members to our vaccinated list. We continue to educate and encourage all staff members to get vaccinated, but those who are unvaccinated will continue to be tested weekly.

Thank you for your patience during the replacing of the roof. We are in the process of adding solar panels over the next few months. This project will be done in phases and we will make sure to communicate any changes to you on a weekly basis.

Dear Family and Friends,

We are happy to report that after two consecutive rounds of testing, all residents and staff have tested negative, keeping our COVID case count to 1 Per Diem employee.

This means that visitation, activities and dining can resume for the unaffected units in the facility (rooms 16-56). The residents on the exposed unit, Station One Low (rooms 1-14), will remain in a temporary quarantine until August 2nd. This will be the protocol as we move forward with the presence of the more contagious Delta variant and experience breakthrough cases.

Today we were informed that a member of our per-diem nursing staff tested positive on a rapid COVID antigen test. We have completed one full round of testing of our residents and all tests have come back negative. We are in the process of testing all employees, and all who have been tested thus far have been negative.

On Monday we will conduct our second round of testing. Unfortunately, we are required to put a temporary pause on our visitation, dining and activity programs.

Once the second round of testing has been completed, we will communicate any changes or updates to you via Constant Contact.

Happy Friday on this hot and humid day. We continue to remain COVID free here at Imperial. Today, we are hosting another vaccine clinic to further protect our residents and staff against the virus. In an attempt to take all precautions against the new Delta variant, we will be instituting a new policy starting Monday, July 19th. All staff members, regardless of vaccination status, who travel out of state, will be tested for 7 days upon returning to work. Please understand that this for the safety of everyone.

On Monday, July 19th, we will be kicking off our Imperial Healthcare Olympic Games. The residents and staff will be divided into two teams and will face off in a variety of competitions throughout the week. We are happy to report another week of activity here at Imperial Healthcare. We continue to hold indoor and outdoor visitation. Please be mindful of the heat index. If the heat index rises above 80 degrees, we will be moving the visits, to designated areas inside of the facility. Please remain in your designated areas until the Activity Department accompanies you back to the lobby area.

On Monday, June 28th the Activity Department opened the Imperial Emporium where residents can use their Royal Rewards Card to spend points that they accumulate while attending activities. I am happy to announce that we are still completely COVID free and are in full swing with dining, activities and beauty services. We have seen a lot of smiles this past week!
 
I am honored to introduce Benedict (Ben) Po, our new Director of Nursing. Although Ben is new to the role of Director, he is not new to Imperial.   1. According to state guidelines, masks are still required in healthcare settings. Please enter the facility with your mask on. We can provide you with a mask if you do not have one.

I would like to take this opportunity to introduce myself. My name is Yochanon Goldstein and as of Monday, May 24th, I am the new Administrator here at Imperial Healthcare. I am excited to join this very talented and dedicated team. Over this week I have had the pleasure of meeting many of the residents and I look forward to meeting many of you as you come to visit your loved ones.
 
I am happy to report that we remain outbreak free and have no new COVID news to report. We will be holding a vaccination clinic on Friday, June 4th to give Pfizer 2nd doses to those who already received their first. Any new admission or employee that would like to be vaccinated can contact the Nursing Department for more information.
 
We had a few other exciting firsts this week. We were very happy to welcome the residents back into the day rooms and dining rooms for meals and activities. It was great to see everyone together and socializing once again.

I would like to formally introduce myself to you. My name is Marc Schon and I served in the role of Administrator here at Imperial Healthcare from 1999 to 2006. Since 2006 I have served in the role of Executive Director overseeing the operational aspects of this facility as well as our other two facilities, The Gardens at Monroe and Foothill Acres. I am excited to be back here in the facility and I look forward to getting to know all of the residents and the family members.
 
I would like to take a moment to applaud the staff at Imperial Healthcare for their dedication and hard work over this last year. Despite the challenges of COVID, the staff at Imperial Healthcare has once again achieved a Five Star status by the Centers of Medicare and Medicaid. This is no small feat and is a direct reflection of the care and caring of the Imperial Healthcare team.
 
I am happy to report that we have zero COVID cases at this time. We continue to test our staff twice, weekly and we continue to implement and maintain the highest of infection prevention methods in the facility.

Dear Family and Friends,
Our weekly Covid update includes two staff members. One of the two employees will be cleared to return to work tomorrow. We have ZERO positive residents at this time.
 
We are happy to report that our pharmacy will be be able to distribute the vaccine to our residents and staff in the near future. We have another large group of employees who will be vaccinated along with a small group of residents. These additional residents will bring our resident vaccination percentage to just under 100%.
 
We understand that everyone’s biggest question is when can indoor visitation resume. The answer is as long as the remainder of Monday’s PCR tests and today’s rapid tests come back with a negative result, we will be able to resume indoor visits on Tuesday, March 30th. If we confirm no new positive cases, a link will be sent on Monday for you to schedule your INDOOR visit.
 
However, we are happy to announce that tomorrow we will begin outdoor visitation, weather permitting. Please use the link at the bottom to schedule your OUTDOOR visit. If you already have a window visit scheduled for tomorrow, it will now become an outdoor visit as long as the resident consents. Please note that residents on a 14 day precautionary isolation are not eligible for a visit until their 15th day.
 
Visits are still limited to two visitors per visit and one visit per week, per resident. EVERY visitor must arrive 15 minutes prior to your scheduled time to be properly screened, given proper PPE and be rapid tested. Rapid testing is not required, but we are asking all families to consent to testing as it will greatly reduce the risk of future COVID spreads within the facility.

During our weekly testing protocols, we learned of one additional staff member testing positive for COVID-19. We will continue the mandatory resident and employee testing.
 
This gives us a total of 0 residents and two staff members positive for COVID-19
 
For the next week we will be adding a third day of employee testing. We have taken this additional measure upon ourselves in order to mitigate any additional spread of the virus.
 
We understand that there are many questions as to the level of infection in our area and how that impacts visitation. Please take a look at the link below to better explain the CALI score and how those decisions are made by the state and local Departments of Health.
 
 
We are considered part of the Central East Region which, as of the last report, still had us listed as high for activity level. Unfortunately, at this time, only end of life visits are allowed.

Today, we were informed by an employee that they have tested positive for COVID at an outside location. They immediately informed nursing administration. This employee has not had contact with any residents for over a week. Due to the last positive employee, we are already in the process of testing all residents so no further steps need to be taken at this time.

We are happy to announce that we have recovered all of our COVID residents at this time. We have 2 employees that are nearing the end of their quarantine period. We did another round of testing of our residents and staff and if all results come back negative, then we can pause the testing of our residents for now. We will continue to test all employees twice, weekly.
 
We had a very successful COVID vaccine clinic today at Imperial Healthcare. Please follow us on Facebook and Instagram to see some of the picture of our Healthcare Heroes taking their first steps towards immunity! Our residents were great patients as they also got their vaccinations. Walgreens will be returning to Imperial Healthcare on January 27th for the second dose of vaccination. Any resident or employee that was not able to be vaccinated today or would like to get the vaccine may do so at that time.

Dear Family and Friends,
Our mandatory weekly testing of employees revealed that we have two positive cases. Both employees will be out of the facility to complete their quarantine. This gives us a total of 3 employees and 2 residents to date. All residents are being tested again today and staff will continue to be tested twice, weekly.
 
Unfortunately, this keeps us in Phase Zero. We will continue to allow closed window visits for the holidays. All visits must be scheduled through the Activity Department by emailing imperialnjcommunications@gmail.com. We will also accommodate Hospice visits for end of life situations.
 
We have been informed by Walgreens that our COVID vaccine clinic is scheduled for Wednesday, January 6th. All residents and family members have been contacted for consent. Please contact the facility if you have any questions regarding the vaccine administration process.

Our mandatory weekly testing of residents revealed that we have two positive cases. Both residents are asymptomatic and have been isolated in the COVID section of the facility. Our mandatory employee testing results revealed that we had one additional employee test positive for COVID. In total we have 2 residents and 2 employees. all asymptomatic.

Our mandatory employee testing results revealed that we had one employee test positive for COVID, but is asymptomatic. Through contact tracing we believe this to be the result of a community transmission. This employee does not provide direct care to our residents, however, in accordance with State guidelines, we will begin with two rounds of resident testing beginning today and another on Saturday.

Dear Family and Friends,
We are pleased to report that we have recovered all of our COVID positive patients at this time. We do still have one asymptomatic employee with 2 remaining days of quarantine. We have now completed two full cycles of resident testing with all negative results. We remain in Phase zero until the local Department of Health officially ends the outbreak.
 
Today, Monmouth County reported a positivity rate over 10%. This means that we will now be testing our staff members twice a week. Staff was tested yesterday and will be tested again this Thursday, December 17th.
 
We are pleased to be able to resume our window visits beginning today, December 15th. Please email imperialcommunicationsnj@gmail.com to schedule your visit. You must report to the lobby to let us know you have arrived.
 
Tomorrow, Wednesday, December 16th, we will host another Family Conference call at 12 PM. Please join us as we will discuss the vaccination information that has been provided to us thus far.

Dear Family and Friends,
Currently, we have one resident on our COVID wing. This resident is stable and asymptomatic. They have also tested negative at least twice since their initial positive test, but will complete the quarantine period on the COVID unit. We have recovered 7 residents to date. All residents were tested on Sunday and again today. We are still awaiting some results from the lab.
 
As per New Jersey Department of Health guidelines, we continue to test our employees every other day until December 14th. From this testing we had one additional staff member test positive giving a total of 2. This employee has no direct contact with residents, but will complete their quarantine at home. The other employee is scheduled to return to work on December 11th. We have had a total of 5 employees recover and return to work.
 
On December 15th we will return to weekly, PCR testing of our staff and will continue to update you on any additional cases.

On Monday, December 1st as per the most recent New Jersey Department of Health Directive, we began rapid testing all employees. Testing will now occur every other day. This testing revealed one new positive result and the employee was sent home to begin quarantine.
 
Today, at 12 PM, we were notified by the lab that one additional resident tested positive. The resident has been isolated on the Covid Unit and the family has been notified.
 
As of now, we have a total of 3 positive residents and 4 employees who are all asymptomatic. We have recovered 5 residents and 2 staff members.

On Tuesday, November 24th, we conducted another round of mandatory testing of all residents and staff members. Additional results have informed us that we have , two additional employees testing positive for COVID-19 and one additional resident. The employees and the resident are asymptomatic.
 
This gives Imperial a total of 5 employees and 6 residents who have tested positive for Covid-19 as one resident has recovered. All staff members are quarantining at home and all residents have been isolated. All residents and employees are asymptomatic at this time.
 
On Wednesday, November 25th, representatives from the New Jersey Department of Health conducted a one day, Infection Control survey. They examined all aspects of infection control and spent time observing caregivers on the COVID-19 unit. They had zero findings and awarded us a second deficiency free survey.
 
Although we are extremely proud of the hard work done by the Imperial Team, we feel that we can still take additional measures to assist in mitigating the spread of this virus. Therefore, we have increased the housekeeping and sanitizing of all common areas including nurses stations, break rooms, med carts, door knobs and restrooms. We will also be reviewing hand washing, PPE usage and all other infection control measures with all staff.
 
We continue to follow all CDC, CMS and State Department of Health guidelines. We have been conducting additional, interim rapid testing in between our weekly testing requirements when necessary. We will complete another round of PCR testing of all staff Monday and Tuesday.
 
Unfortunately, no visitation is allowed at this time except to accommodate those in an end of life or compassionate care situation. On Thursday, November 19th, we conducted our second round of testing of all residents and staff members to establish a new baseline. From that round of tests, one additional employee tested positive for COVID-19. We immediately began testing any resident that the employee has come in contact with. The rapid testing of those residents revealed one additional resident who also tested positive. This gives Imperial a total of 2 employees and 6 residents who have tested positive for Covid-19. Both staff members are quarantining at home and all residents have been isolated. All residents and employees are asymptomatic at this time.

Today we were notified by our lab that we have five additional positive COVID-19 cases including four residents and one employee giving us a total of 5 residents and one employee. The residents have been isolated and the employee is quarantining at home. All six cases are asymptomatic.

During our routine, weekly testing, we were notified of a positive COVID-19 result in one of our long-term care residents. At this time, the resident is asymptomatic and has been placed on isolation.
 
All residents and staff will be tested within 24 hours and then a second round of testing will be completed on Thursday to establish a new baseline. Unfortunately, we will have to suspend all indoor, outdoor and window visitation until further notice. On Wednesday, October 28th, I signed and submitted the following attestations:
  • Implemented all the requirements set forth in Executive Directive 20-026¹
  • Not experienced any new facility-onset of COVID-19 cases in 14 days,
  • has a mechanism to collect informed consent from the residents and visitors,
  • has a location designated for indoor visitation,
  • sufficient staff, a mechanism for appointments, and sufficient PPE and cleaning and disinfection supplies to permit visitation.
These new guidelines will allow us to move to Phase Two which will start the beginning of indoor visitation on Tuesday, November 3rd. We will start with a modified visitation schedule for indoor visitation, but we will continue outdoor visitation as well. *Please note that each resident will be limited to one type of in-person visit per week. All visits are still limited to two family members per visit. You must arrive 10 minutes prior to your visit to complete the screening process. All visits will end as scheduled regardless of the time of arrival to accommodate the cleaning and disinfection process prior to the next scheduled visit.
 
 
Please note that all visitors will be expected to adhere to the following guidelines:
 
NJ Visitor Policies at Long-Term Care Facilities
A resident who is suspected or confirmed to be infected with COVID-19 or quarantined for a COVID-19 exposure cannot have visitors. A resident who has tested positive is allowed visitation after they have met the criteria for discontinuation of isolation.
·      Both visitors and residents must wear face coverings.
·      Long-term care facilities should communicate the visitor policy to residents, families, staff and others. They should receive informed consent from the resident and the visitor in writing that they are aware of the possible dangers of COVID-19 exposure and that they will comply with the facility’s policies during the visit. As part of the consent form, the visitor must agree to notify the facility if they test positive for the virus or have symptoms within 14 days of visiting.
·      Upon screening, facilities must prohibit visitation for those who meet one or more of the following criteria:
1) Exhibit signs or symptoms of an infectious communicable disease, including COVID-19, such as a subjective and/or objective fever (evidenced by a temperature check of the visitor equal to or greater than 100.4 F or as further restricted by facility), chills, cough, shortness of breath or difficulty breathing, sore throat, fatigue, muscle or body aches, headache, new loss of taste or smell, congestion or runny nose, nausea or vomiting, or diarrhea;
2) In the last 14 days, has had contact with someone with a confirmed diagnosis of COVID-19, or someone under investigation for COVID-19, or someone ill with respiratory illness.
3) In the last 14 days, has returned from a designated state under the 14-
day quarantine travel advisory; or
4) Has been diagnosed with COVID-19 and has not yet met criteria for
the discontinuation of isolation per guidance issued by NJDOH and CDC.

I am happy to report that we remain COVID free at Imperial Healthcare. On Wednesday, October 14th, we entered Phase One. This phase allowed us to begin small group activities and small group dining. Residents were placed into two groups. One group participates in morning activities and enjoys lunch in the dining room and the second group attends afternoon activities and stays for the evening meal. It has brought a smile to my face to see our residents out and socializing with our staff and each their friends once again.
 
I will be signing the attestation to proceed to Phase Two by the end of the week. This would allow us to begin Phase Two sometime next week. Phase Two will allow for the scheduling of indoor visits. We are very happy to announce that we have been approved to move to Phase One of the Action Plan outlined by the Centers for Medicare and Medicaid Services and the New Jersey Department of Health.
 
On Friday, October 9th, I attested to the following requirements;
  • End of Outbreak
  • PPE Emergency Use Stockpile
  • Data Reporting on NHSN twice a week
  • Infection Prevention Contract
  • Phased Reopening.
 
Today, we will begin small group activities of 10 or less while following all CDC guidelines with social distancing and mask wearing. These groups will remain consistent to limit the chances of virus spread across multiple residents. Soon after, we will begin small group dining. Please understand that we are trying to accommodate all of our residents and so we ask for your patience while we work through this phase.
 
Our next goal is to move to Phase Two within the next few weeks. Phase Two will allow us to scheduled indoor visits. For now, please continue to schedule your outdoor and Skype visits through the Calendly site and the Activity Department. Imperial Healthcare continues to remain COVID free, but should there be a confirmed case you will be notified by 5PM the following day via Constant Contact and it will be posted to our website. Please take a moment to review these reminders for scheduling your weekly, outdoor visits. These rules and regulations have been set forth by the New Jersey Department of Health to ensure safe visits for all involved.
 
  • Each resident is only allowed one outdoor visit per week in order to give each resident an opportunity for a weekly visit. 
  • Please remember that only two family members are allowed per visit. Any other family members will be asked to wait in their vehicle. 
  • You MUST arrive ten minutes prior to your scheduled visitation time in order to complete the paperwork and have your temperature taken.

In order to move forward to the next phase of reopening, our team at Imperial Healthcare is working hard on implementing the new guidelines released by the Department of Health on August 10th .One of the requirements is that all communications, data and information regarding COVID-19 must be displayed on our website.   • PROCEDURE
    Choosing Essential Caregivers
    1. All residents may receive Essential Caregiver visitation, except for those residents in a 14-day
    quarantine period, positive for COVID-19, and have not yet met the criteria for the
    discontinuation of isolation.
    2. Imperial Healthcare staff members must consult the resident or resident’s representative (i.e.,
    Power-of-Attorney) about the resident’s wishes or best interests to determine whom to
    designate as the Essential Caregiver.
    3. Those interested in serving as Essential Caregivers must complete the “Imperial Healthcare
    Essential Caregiver Application.” Administration in collaboration with the Director of
    Nursing, will approve or deny the application to serve as an Essential Caregiver.
    4. Both the resident and Essential Caregiver must sign the “Imperial Healthcare Visit Rules,
    Informed Consent, Hold Harmless, & Release Regarding COVID-19” and “Essential
    Caregiver Agreement” prior to beginning the Essential Caregiver relationship.
    5. Residents or residents’ representatives may express a desire to designate more than one
    Essential Caregiver based on their past involvement and needs (e.g., more than one family
    member previously split time to provide care for the resident). In these unique situations,
    Imperial Healthcare staff members will work cooperatively with the resident and family to
    work out a schedule to accommodate the Essential Caregivers. A resident may have no more
    than two (2) Essential Caregivers.
    6. Essential Caregivers must be at least eighteen (18) years old.
    7. Essential Caregivers must plan to visit residents at least once per week, absent exceptional
    circumstances or community prohibitions on entry.
    Role of Essential Caregivers
    8. Essential Caregivers will support and encourage residents with activities of daily living
    (ADLs) and have an essential purpose.
    9. Essential Caregiver visitation is not solely to provide social visits.
    10. Imperial Healthcare staff members will hold a care conference and careplan the role of each
    Essential Caregiver.
    Testing & Screening Essential Caregivers
    11. For the safety and wellbeing of the residents and employees, Imperial Healthcare strongly
    recommends that Essential Caregivers demonstrate results of two negative COVID-19 tests
    before providing care to the residents: The first negative COVID-19 test should occur seven
    (7) days prior to the second test. The second test should occur no more than five (5) days prior
    to the visit at Imperial Healthcare. The Essential Caregiver should thereafter continue to
    demonstrate negative COVID-19 tests on a weekly basis or two (2) negative COVID-19 test
    prior to returning to provide care at Imperial Healthcare.
  • 12. Essential Caregiver can undergo COVID-19 testing through their own healthcare providers.
    Essential Caregivers also have the option of undergoing COVID-19 testing at Imperial
    Healthcare by providing valid insurance information; Essential Caregivers are responsible for
    the unpaid costs of any COVID-19 testing at Imperial Healthcare.
    13. Essential Caregivers will always be screened prior to being allowed access into Imperial
    Healthcare. Screenings shall include temperature checks, questioning on symptoms and
    diagnosis of disease (including COVID-19), and questioning on travel and interaction with
    those with COVID-19 and suspected of having COVID-19.
    14. For the safety and wellbeing of the residents and employees, Imperial Healthcare strongly
    recommends that Essential Caregivers provide proof of having received a flu vaccine to
    Imperial Healthcare by November 1, 2020.
    15. Essential Caregiver visits will be scheduled in advance. Essential Caregivers may visit during
    the following hours: Monday through Friday, 9am-4pm, and Saturday-Sunday, 10am-4pm. To
    reduce congestion during the screening process, Essential Caregivers may not enter the facility
    during the changes of shift.
    16. Essential Caregivers must enter the facility through the main entrance and must sign in and
    out of the facility for each visit.
    17. When Imperial Healthcare is in the Red Zone (Phase 0 pursuant to NJ DOH guidance), a
    resident may have Essential Caregiver care one (1) visit per week for a maximum of two (2)
    hours (per visit).
    18. When Imperial Healthcare is in the Orange Zone or Yellow Zone (Phase 1 or Phase 2,
    respectively, pursuant to NJ DOH guidance), a resident may have Essential Caregiver care for
    two (2) visits per week for a maximum of two (2) hours (per visit).
    19. When Imperial Healthcare is in the Green Zone (Phase 3 pursuant to NJ DOH guidance),
    regular visitation will resume at Imperial Healthcare.
    20. Residents must receive care from their Essential Caregivers in areas designated for such
    visitation by Imperial Healthcare administration.
    21. Essential Caregivers must remain in their assigned areas for their visits.
    22. Essential Caregivers must engage in hand hygiene and social distancing from others besides
    the resident while in the facility.
    23. The Essential Caregiver must maintain social distancing of at least six (6) feet with staff and
    other residents while I the facility.
    24. Imperial Healthcare staff may stop any Essential Caregiver visit or visits if it is believed that
    the visit or visits are not in the best interest of the resident or the Imperial Healthcare
    community.
    Personal Protected Equipment (PPE)
    25. The Essential Caregiver must always wear proper Personal Protective Equipment (PPE) while
    in the facility. Depending on the NJ DOH Phase, this PPE could include a mask, face shield,
    and isolation gown. Essential Caregivers must always wear masks while in the facility.
    26. Essential Caregivers must provide their own PPE.
  • 27. Essential Caregivers must watch the Centers for Disease Control and Prevention (CDC) video
    on donning and doffing PPE (available at https://www.youtube.com/watch?
    v=1xy00pLT9M4&feature=youtu.be ) and sign the “Attestation – CDC Sequence for Donning
    and Doffing Personal Protective Equipment (PPE)” that they have watched same.
    RESPONSIBLE PARTIES
    1. On an annual basis, our commitment to and implementation of this policy shall be reviewed.
    2. Imperial Healthcare administration is responsible for the implementation and communication
    of this policy.
    3. Imperial Healthcare clinical management and Infection Control Preventionist are responsible
    for monitoring and reporting on the implementation of this policy. This responsibility includes
    bringing to the attention of the administration team instances where this policy is not being
    consistently applied.

Ensure sufficient supply of Hand Hygiene resources in the facility.

 

Evaluate personal protective equipment (PPE) and report level to corresponding agency/OEM/LDOH/NJDOH.

 

Educate staff on infection prevention practices, control measures for COVID-19.

 

Facility will implement Universal Source control by providing facemask to person in the facility. As an alternative, cloth mask will be provided in the event of supply shortage for staff that does not provide direct care to the residents.

Admission, Transfer and Readmission

 

Consider closing to new admissions if the facility is unable to appropriately cohort. This does not include readmission back to the facility.

 

Close the unit affected to new admissions except as needed to cohort individuals and staff.

 

The facility will coordinate with the transport company if a presumptive or confirmed COVID-19 is going to be transported. Face mask will be provided to the resident for transport to protect the transport staff and receiving facility staff.

 

The facility physician will coordinate with the receiving facility’s physician to ensure report is accurately communicated and precaution is in place.

 

Dedicate equipment in isolation rooms. If not possible, clean and disinfect equipment using the EPA-registered disinfectant before use with another resident within the cohort.

 

Conduct routine cleaning and disinfection of high touch surfaces and shared medical equipment using EPA-registered disinfectant.

 

Increase frequency of routine cleaning and disinfection.

 

Prioritize rounding in a “well to ill’ flow to minimize cross-contamination.

 

 

 

 

New admissions and readmission will be monitored for signs of COVID 19 for 14 days and will be cared for by staff using COVID-19 PPE according to CDC guidelines. They will be placed in private rooms (if available). If feasible, a cohort will be created for this population.

 

 

Emergency Staffing Strategies:

POLICY:

 

In the event of an emergency, the Administrator or DON will make the decision to utilize emergency staffing strategies as necessary to provide for care and treatment of residents. There is a staffing contingence in place. Please refer to the policy and procedure during an emergency.

 

 

Virtual Communication:

 

Policy:

To continue to provide the vital opportunity for family members to interact with their loved ones while minimizing the chance of the spread of COVID-19.

Procedure:

  • Scheduling for all video chats will be done by the Activity Department.
    • All family members were notified to send an email for all virtual visitation requests to Imperialnjcommunications@gmail.com at least 24 hours prior to requested video chat time.
    • Email addresses for new admissions are obtained by Admissions and/or Receptionist.
    • Chats are scheduled daily from 11 am to 4:30 pm and are limited to 10 minutes.
    • A daily schedule will be printed and distributed to each unit and Activity Supervisor so everyone knows what chats are scheduled for the day.
    • Activity Director and/or Activity Supervisors will coordinate who will be assigned to carry out each video chat scheduled.
      • On units with suspected or confirmed COVID-19 or 14-day quarantine, the Activity Supervisor or Director will coordinate with Unit Managers to identify any residents that could done by Nursing staff while in the rooms.
    • When video session is complete, staff is to log the results and notify Activity Director or Supervisor of any issues during the call.
    • All tablets and iPads will be wiped down with disinfecting wipes before and after each use.

 

Infection Prevention and Control

Restrict visitors and non-essential healthcare personnel, except in certain compassionate-care situation.

 

Evaluate all persons who enter the facility for signs of COVID-19. Temperature checks at entry point. Temperature checks at the start of each shift for all employees.

 

The facility will monitor any visitors/vendors/physician for temperature and any signs of respiratory symptoms. The following will also be assessed: history of international travel with 14 days, history of exposure to a presumptive or positive COVID-19.

 

Visitors with respiratory symptoms will not be permitted to enter the facility at any time.

 

Facility will require visitor who are cleared to enter the facility will be asked to perform hand hygiene and use of facemask and restrict visitation to resident’s room.

 

Signage for visitation, cough etiquette and hand hygiene will be posted in several location within the facility.

 

Visitation restriction will be communicated through email, website and posting of signage

 

Active screening for residents for fever and other COVID-19 symptoms every shift.

 

Any supply delivery will be dropped off by the loading dock and/or right by the reception area.

 

Alcohol-based hand sanitizers for hand hygiene will be provided in the hallways, in each resident’s room, medication carts and throughout the building.

 

 

PPE needed will be made available outside the doors for easy access to the staff.

 

Facility will provide EPA disinfectants to allow for frequent cleaning of high touched surfaces and shared resident equipment.

 

Suspend communal dining and all group activities.

 

Residents are encouraged to remain in their rooms. If the resident leaves their room, they should wear a face covering or face mask; perform hand hygiene, limit movement in the facility and perform social distancing.

 

Waste receptacles will be provided for PPE

 

Implement Standard and Transmission-Based Precaution including use of N95 mask.

Resident Management / Cohorting

 

Implement cohorting plan that allows for separation of residents, dedicating staff and medical equipment to each of these cohort and allowing necessary space to do so at the onset of an outbreak.

 

In the event staffing cannot be designated specifically in the COVID-19 unit, Staff will work from clean unit to COVID Unit., Staff will cluster work load from clean resident to COVID residents.

 

Staff will don COVID-19 recommended PPE according the CDC guidelines.

 

Laboratory confirmed COVID-19 positive residents will be relocated to designated cohort. Private rooms will be provided if feasible. Cohort with another COVID-19 resident maybe done when private room is not possible.

 

Person Under investigation will not be moved to COVID-19 cohort until results are confirmed.

 

Roommates of COVID-19 residents is considered exposed and should not share a rooms with others unless they have tested negative and has been asymptomatic for 14 days from last exposure.

Facility will adhere to the CDC guideline for the discontinuation of Transmission-Based Precaution.

Symptom-based strategy

At least 3 days (72 hours) have passed since recovery defined as resolution of fever without the use of fever-reducing medications and improvement in respiratory symptoms (e.g., cough, shortness of breath); and,

At least 10 days have passed since symptoms first appeared

 

 

Staff Management

All residents will be provided mask and instructed to cover their nose and mouth when around others, as tolerated.

 

Enforce use of universal facemask (source control)for staff while in the facility in addition to active screening of symptomatic staff.

 

Staff will be screened by getting their temperature taken and completing a screening tool at the start of their shift

 

Any staffs that shows symptoms consistent with COVID-19 will be asked to put on mask and leave the facility. Such staff would require clearance from IP before returning to work.

 

Identify staff who maybe at higher risk for severe COVID-19 and attempt to assign to unaffected units.

 

Educate and reinforce sick leave policy including not to report to work when sick.

 

Assess staff competency on infection prevention and control measure including putting on and taking off PPE.

 

Conserve use of PPE using CDC guidance for PPE contingency strategies.

The facility will monitor any visitors/vendors/physician for temperature and any signs of respiratory symptoms. The following will also be assessed: history of international travel with 14 days, history of exposure to a presumptive or positive COVID-19.

 

Visitors with respiratory symptoms will not be permitted to enter the facility at any time.

 

Facility will require visitor who are cleared to enter the facility will be asked to perform hand hygiene and use of facemask and restrict visitation to resident’s room.

 

Signage for visitation, cough etiquette and hand hygiene will be posted in several location within the facility.

 

Visitation restriction will be communicated through email, website and posting of signage

 

Active screening for residents for fever and other COVID-19 symptoms every shift.

 

Any supply delivery will be dropped off by the loading dock and/or right by the reception area.

 

Alcohol-based hand sanitizers for hand hygiene will be provided in the hallways, in each resident’s room, medication carts and throughout the building.

 

 

PPE needed will be made available outside the doors for easy access to the staff.

 

Facility will provide EPA disinfectants to allow for frequent cleaning of high touched surfaces and shared resident equipment.

 

Suspend communal dining and all group activities.

 

Residents are encouraged to remain in their rooms. If the resident leaves their room, they should wear a face covering or face mask; perform hand hygiene, limit movement in the facility and perform social distancing.

 

Waste receptacles will be provided for PPE

 

Implement Standard and Transmission-Based Precaution including use of N95 mask.

Resident Management / Cohorting

 

Implement cohorting plan that allows for separation of residents, dedicating staff and medical equipment to each of these cohort and allowing necessary space to do so at the onset of an outbreak.

 

In the event staffing cannot be designated specifically in the COVID-19 unit, Staff will work from clean unit to COVID Unit., Staff will cluster work load from clean resident to COVID residents.

 

Staff will don COVID-19 recommended PPE according the CDC guidelines.

 

Laboratory confirmed COVID-19 positive residents will be relocated to designated cohort. Private rooms will be provided if feasible. Cohort with another COVID-19 resident maybe done when private room is not possible.

 

Person Under investigation will not be moved to COVID-19 cohort until results are confirmed.

 

Roommates of COVID-19 residents is considered exposed and should not share a rooms with others unless they have tested negative and has been asymptomatic for 14 days from last exposure.

Facility will adhere to the CDC guideline for the discontinuation of Transmission-Based Precaution.

Symptom-based strategy

At least 3 days (72 hours) have passed since recovery defined as resolution of fever without the use of fever-reducing medications and improvement in respiratory symptoms (e.g., cough, shortness of breath); and,

At least 10 days have passed since symptoms first appeared

 

 

Staff Management

All residents will be provided mask and instructed to cover their nose and mouth when around others, as tolerated.

 

Enforce use of universal facemask (source control)for staff while in the facility in addition to active screening of symptomatic staff.

 

Staff will be screened by getting their temperature taken and completing a screening tool at the start of their shift

 

Any staffs that shows symptoms consistent with COVID-19 will be asked to put on mask and leave the facility. Such staff would require clearance from IP before returning to work.

 

Identify staff who maybe at higher risk for severe COVID-19 and attempt to assign to unaffected units.

 

Educate and reinforce sick leave policy including not to report to work when sick.

 

Assess staff competency on infection prevention and control measure including putting on and taking off PPE.

 

Conserve use of PPE using CDC guidance for PPE contingency strategies.

 

Consider staffing contingency for shortage (refer to separate policy).
